TSB finds many factors led to Barrhaven bus-train crash

OTTAWA – The Transportation Safety Board says driving distractions, speed and roadway configuration were among the factors that led to a deadly 2013 Barrhaven bus-train collision.
In its final report on the crash, the board says 15 factors contributed to the crash, which killed six people including the bus driver.
Lead investigator Rob Johnston says that, given all the factors, the accident could have happened to any driver.
The investigation concludes that the driver’s view of the road was obstructed by foliage as the bus approached the crossing just as the train was coming into its path.
To address what it calls major safety deficiencies identified during the investigation, the board makes five recommendations, including changing the in-vehicle displays used in the buses.
